Documentation Guidelines

Eligibility for services will be determined based on the documentation you provide to DSP&S that verifies your disability or condition.   The documentation must specify:   

• Disability/ Condition   
• Be signed by your verifying professional who can diagnose the disability/ condition   
• Must identify educational limitations that keep a student from fully participating in general education without additional specialized services.   

Documents and Forms Needed  

We will be asking for the most recent, signed, and dated documentation such as, an IEP and Psycho-Educational Report.   

If you currently have a physical disability or psychological condition you may provide the Eligibility Verification Form to your licensed practitioner and upload or submit it to the DSP&S office (Bldg. 2900).  

To Return/ Upload your Documentation  

• All documents must fax (760-355-6434) or upload.  
• Please title your fax "New Student” and include your G#.   
• When your documentation is received, it will be added to your file at DSP&S. The review process may take up to 14 working days.   
• If approved, you will be notified through your IVC student email.   

PLEASE NOTE: If there is an increase in the applications received the review process may extend for up to four (4) weeks. It will be the student’s responsibility to keep in mind the turn-around time when trying to meet semester deadlines.
